2009 Fall Preseason Registration Now Available
Welcome to the registration page for the 2009 fall athletic
season. Below you will find three links, one is the Athletics Registration Form, one
is the Student
Health Record Form and the final one is the Returning
Athlete Sports Participation Assessment. All three of
these forms must be filled out prior to your arrival at preseason
camp. The athletic registration can be submitted online. The health
record and Returning Athlete Assessment must be printed and
mailed back to the following address:

* Failure to fill out proper documentation prior to your arrival to
preseason will result in dismissal from camp.
The deadline for Football registration is August 11th.
The registration deadline for all other fall sports is August 18th.

Are you ready to compete?
(Requirements for Athletic Participation)
- Cleared by the business office
- Academically and athletically eligible
- Proof of current health insurance
- Enrolled as a full-time student (Min. 12 credits)
- Satisfactory progress toward a baccalaureate degree (return/transfer students)
- Signed Acceptance of Risk document
- Sign NCAA forms prior to competition
- o Student Athlete Statement
- o Drug Testing Consent
- Proof of physical exam

Students without a current physical will not be allowed to stay for pre-season camp.
- Signed sports information release form
- Transfer students:
- o Transcripts from all previous colleges or universities
- o Eligibility approved by last institution attended
